Please...

If you are unable to keep an appointment,

we ask that you kindly provide us with at least 48 hours notice. This courtesy, on your part, will make it possible to give your appointment to another patient. Co-payments and co-insurances are due at the time services are rendered. Forms of payment accepted by the office are check, cash, insurance assignment as well as most major credit cards. Our office provides on-call service for emergencies, 24 hours-a-day, 7 days-a-week.

Our Vaccination Policy

We are committed to providing the highest standard of care and protecting the health of all our patients. Vaccination plays a crucial role in safeguarding children and our community against serious diseases.

While we understand that families may have questions or concerns, we do require that all patients follow a vaccination schedule. We are happy to support a delayed schedule if needed; however, we ask that the first vaccine be administered by the time your child is 2 months old.

If you decide not to vaccinate, we may not be the right fit for your family, and we would respectfully ask you to seek care from another practice that aligns better with your views.
